In typical Donna fashion, it has drawn her up right to a stop where she stands, not mindful at all that others might be around. But no. No. There is no way he has a village inside... Or maybe he does? She shakes her head though, as it isn't going to help her. "Oh, great. Now you're going to start talking to yourself Donna, only you can't give yourself the answers. I'm going to smack that dunce when I get my hands on him."
And as it happens, she does turn, moving in the direction that he is in.

Hummm... Well, he could pretend to be Caecilius.
"I always found talking to yourself as a way to keep sane."
He thinks to himself: Good one, Doctor, because that doesn't sound like you at all.

"Caecilius?" She hasn't expected to see him again... "But, what are you doing here? How are you here? Is your family here?"

Well, two out of three isn't bad. He knows he really didn't have anything to do with it.

"Well, after you dropped us off in Rome, things got a lot better for us and I never quite forgot the way Doctor Spartacus dressed."
He hopes his 'making it up skills' are working well enough, or he's in trouble. He offers a grin to cover up his fears.
But... there's one more clue, if Donna should notice it. His clapping hand. It's always going in a steady but calm rhythm.

Because yeah, this is just how the Doctor rolls now. And that hand-clapping is still going on...

Which is a lie. But he's afraid to tell her who he is beyond this.

He offers a small shrug. A gesture that might actually seem familiar to Donna.
"Maybe we could discover it together."

"That is pretty much always what it is to me. Why do you think it's more?"
Obviously, he's not saying she's wrong.

And, oh, forget it! "Come on, if you're coming. Let's go see what's about. There has to be something."

"I have missed you, Donna Noble."
And there might be a tone of familiarity there that Caecilius never had with Donna (and never would, no offense). But more than that it would sound rightly proper if it came out of a mouth with an Estuary accent rather than a Scottish one.
"Pick a direction, any direction."
And yes, the hand clapping is still happening.

She ends up picking the route that takes them a bit further in to a populate area. Her brain is turning, thinking, although in the end she ends up just blurting it out. "You're not Caecilius, are you? So the question is, who are you, and why are you wearing his face? If you want something, I don't have anything. My mum will tell you to keep me if you're looking for a ransom."

"I'll answer your questions, if you answer mine first. How's your head? Any piercing pain? Does it feel like exploding? Better yet, what was the last adventure you had with the Doctor?"
Because he had to be sure first. Her safety - her life was worth more than all the pain he would endure of being someone else around her.
